What city is your “home base?”: Maitland
How far are you willing to travel?: Anything further than 20miles from Maitland location has a driving fee.
Do you offer a sliding scale or discounted services?: I can offer a payment plan for in office services. A credit card must be kept on file and a payment plan agreement must be signed and agreed to.
Do you take any insurance plans or medicaid?: I do not accept insurance as a form of payment, but I’m happy to provide a Superbill that you can submit for possible reimbursement. Please note that reimbursement is not guaranteed. Payment with FSA or HSA funds is also accepted.
